Complete the Application then tick the BOX to confirm that you have read and agree with the terms of the Privacy Act. At the end of the form it asks you to upload a copy of your Drivers licence, a copy of your last 2 Pay slips and a copy of the front page only of your Bank Statement, which are all used to Identify you.

A new requirement is for you to upload a copy of your Medicare card and also a picture of the back of your drivers licence.
